Our guiding principle from the start has been: caring is our main concern. We achieve this goal by providing special services such as physical therapy, occupational therapy, respiratory therapy, and wound care. We are a residential facility that provides 24-hour medical care and rehabilitation services for individuals who require a higher level of care than what can be provided at home or in an assisted living facility. At Hillcrest Post Acute we believe that patients come first so we want to treat each patient as if they are the only one.

Hillcrest Post Acute is located in Petaluma, California’s where it was original founded in the 1950s as Hillcrest Hospital, the facility served as Petaluma’s primary medical center until the late 1970s, when it transitioned into the skilled nursing facility. In 2016 it was renovated into a post-acute care and short-term rehabilitation center with a commitment of closing the gap between hospital and home, providing just-right care that nurtures recovery and restores independence. From well-appointed rooms and scenic patios to a caring, highly trained staff, every detail is designed to foster wellness and comfort.
